An small example of a typical conversation:
Therapist: Now try to soften your shoulders
You: They are soft, At least they feel relaxed.
Therapist: They may feel relaxed but look at the screen, you haven't let go, you just think you have. Now let them go while you are looking at the screen so you can tell.

The "Association for Applied Psychophysiology & Biofeedback" Official Definition:
"Biofeedback is a process that enables an individual to learn how to change physiological activity for the purposes of improving health and performance. Precise instruments measure physiological activity such as brainwaves, heart function, breathing, muscle activity, and skin temperature. These instruments rapidly and accurately "feedback" information to the user. The presentation of this information — often in conjunction with changes in thinking, emotions, and behaviour — supports desired physiological changes. Over time, these changes can endure without continued use of an instrument. "(AAPB, 2008)
Standards:
Biofeedback/psychophysiology lends itself to very strict clinical and research standards.
Every session generates hard numerical data. It can easily be printed, reported and evaluated by the therapist, the client and the person who may have referred the client. In practical terms, this means that if the client is or is not improving, the longitudinal data will display the change.
The Association of Applied Psychophysiology and Biofeedback (AAPB) keep a list of efficacy levels and research on applied psychophysiology. There is also an abundance of information about psychophysiology/biofeedback available on PubMed etc.
